智慧醫療政策的發展是近年來城市健康治理之主軸，民眾對於政府在智慧醫療推動的期望與日俱增。臺灣因應慢性疾病比率逐年提升，提出遠距健康照護計畫，各地方政府透過資通訊技術 (Information and Communication Technology， ICT) 與醫療服務的結合，發展智慧化個人健康照護管理系統。衛生福利部自104年8月起陸續與12個地方縣市政府合作，規劃全國遠距健康照護服務網絡，由各縣市建置雲端系統，提供個人化的生理量測資料服務。目前國內研究著重於遠距健康照護理論發展與儀器功能的創新之探討。然，缺乏有系統地探索地方政府實施遠距健康照護服務之經驗，因而無法全面地瞭解臺灣各地方政府的實施經驗現況跟及差異。故，本文提出研究問題：遠距健康照護計畫之發展現況及服務類型差異為何，並探究資訊科技對該政策之影響。透過官方資料蒐集與半結構質性訪談，研究結果發現，參與遠距健康照護服計畫之縣市，部分縣市已終止服務，而影響遠距健康照護計畫成效之關鍵因素有：差異性服務架構、健康資料庫整合、多元合作網絡、民眾醫療習慣等。另外，資通科技的輔助有助於社區型據點建立健康資本網絡。故，遠距健康照護計畫應朝向數據運用與系統整合，提出因地制宜的服務模式。地方政府應將外部廠商、內部跨單位之多元合作模式，納入計畫考量。最後，依據資料分析結果梳理歸納出本研究之結論，並提出政府在遠距健康照護發展之相關建議。

The adoption of innovative healthcare technologies has increased in the decade. Taiwan’s telecare service experience is well-known in the world and has been the example of many countries. The users of telecare service would examine their daily health status, and the service system has the function of providing individuals’ health recode. Most of those users who review their health status and recode would have health maintenance strategies. Much has emphasized on users ‘satisfaction and cost-effectiveness analysis of telecare service. However, current studies have not clearly combed the various types of telecare service and examined their main issues in Taiwan. This research conducted the content analysis of all the telecare service information in websites and examined the government documents of telecare service in order to present the main characteristics of telecare service of Taiwan’s local governments. In addition, this research used semi-structured-depth interviews to collect the views of the telecare service administrative staff and analyze the main issues and further suggestions of telecare service development in Taiwan. This study found that comprehensive system evolution mechanism and service collaboration are vital to the implementation of Telecare Service programs, and government resource support and health policy would have the positive influence on telecare service system development. Importantly, public managers have to think about the method of increasing users’ access to the telecare service.
