政府主管機關規定醫療機構內有醫師、護理人員、藥師等有許多不同類別的專業執照；各類的醫事人員法及醫事人員執業登記及繼續教育辦法，進一步規範了各類醫事人員之執業、繼續教育、異動等變更，皆須向地方醫事公會及主管機關規定申請登記。如此針對醫療機構內各類的醫事人員的嚴格規範，顯見政府為了全民的就醫安全，把關甚嚴。
但對於醫療機構而言，針對院內各類醫事人員需前往地方主管機關進行執業登記，須耗費機構一定的人力及時間成本；尤其對於最高級別的醫療機構，因院內員工中的醫事人員多達八成，各類執照的類別繁多，人事異動的頻繁，更是大幅增加了機構所需耗費的人力及時間成本。
本研究以一間醫學中心為研究對象，收集了該院與地方主管機關在執業登記上的相關資料，並將該院的執業登記作業歸納整理成5種類型（1.新進人員；2.變更院區、科別及外國人執業期限；3.執照更新；4.出國與留職停薪；5.回國與復職）。
首先，將歸類的各項登記流程，細分為18個步驟，描繪出該院申請執業登記流程的完整樣貌。其次，計算整理出該院近三年辦理執業登記總作業量與百分比、作業時間及百分比。第三，歸納整理出該院在辦理執業登記遇到的問題。
根據研究結果，本研究認為對於現今已處於作業效率化與資料數位化的時代，雖然醫事人員的執業登記有其法規上的必要性與保障民眾權益上的必然性，但醫療機構仍可對此一作業，依據現有的做法與數據進行整理，並做出進一步的改善，以增進此一作業的成效。

The competent government authorities require many different types of professional licenses for doctors, nurses, pharmacists, etc. in medical institutions. In addition, various medical personnel laws and the Regulations Governing Practice Registration and Continuing Education of Medical Personnel further regulate medical personnel practice, continuing education, and workforce changes, all of which must be registered with local medical associations and competent authorities. Such stringent regulations on the various types of medical personnel in medical facilities indicate that the government has been acting as a gatekeeper in protecting patients’ safety when they seek medical care.
In medical establishments, however, the need for their medical staff to register practice with the local competent authorities is costly in terms of both human resources and time, especially for the highest level of medical institutions, as 80% of their staff are medical personnel, with a great number of different types of licenses and a high turnover.
This study involved a medical center and collected information related to the practice registration by the facility with the local competent authorities, with the practice registration categorized into five types 1) new recruits; 2) changes in hospital campus, specialty, and foreigner practice period; 3) license renewal; 4) going abroad and leave without pay, and 5) returning home and reinstatement.
In this study, the registration process was first broken down into 18 steps to give a complete picture of the process of the facility applying for practice registration. Second, the total volume and percentage of handling practice registration, the processing hours and percentage by the facility over the past three years were calculated and sorted out. Third, the problems encountered by the center in registering its practice were summarized.
According to the results of this study, in the era of efficiency and digitization, although the practice registration of medical personnel is bound by regulations to safeguard the rights of the public, medical facilities can still organize and make further improvements to existing practices and data in order to enhance the effectiveness of this operation.
